First Time Applicants:

Proof of U. S. Citizenship
Applicants born in the United States: Submit previous U.S. Passport or certified birth certificate. A birth certificate must include you given name and surname, date & place of birth, date the birth was filed &the seal or other certification of the official custodian of such records. If the birth certificate was filed more than 1 year after the birth: It is accepted if it is supported by evidence described in the next bullet. If no birth record exists: Submit registrar's notice to that effect. Also submit an early baptismal or circumcision certificate, hospital birth record, early census, school or family Bible records, newspaper or insurance files, or notarized affidavits of persons having knowledge of your birth. Evidence should include your given name & surname, date & place of birth, and seal or other certification to the issuing office & signature of issuing official.

Proof of Identity
You must establish your identity to the acceptance agent. You may submit items such as the following containing your signature & physical description or photograph that is a good likeness of you: previous U.S. passport, Certificate of Naturalization, Certificate of Citizenship, driver's license (not temporary or learner's license), or government (Federal, State, municipal) employee identification card or pass. If you cannot prove your identity as stated above, you must appear with an Identifying Witness who is a U.S. Citizen or permanent resident alien who has known you for at least two years. Your witness must prove his or her identity & complete & sign an Affidavit of Identity Witness (Form DS-71) before acceptance agent. You must also submit some identification of your own.

Two photographs
Submit two identical photographs of you alone, sufficiently recent to be a good likeness of you (normally taken within the last 6 months), & 2 X 2 inches in size. The image size measured from the bottom of the chin to the top of the head (including hair) should not be less than 1 inch & not more than 1 3/8 inches. Photographs must be clear, front view, full face, taken in normal street attire without a hat or dark glasses, & printed on thin paper with a plain light (white or off-white) background. They may be in color or black & white. They must be capable of withstanding a mounting temperature of 225 degree Fahrenheit (107 degree Celsius). Photographs retouched so that your appearance is changed are unacceptable. Snapshots, most vending machine prints, & magazine or full-length photographs are unacceptable. Digitized photos must meet the previously stated qualifications& will be accepted for use at the discretion of Passport Service. (Visit our website for details).

Fees to one of the following acceptance agents: a clerk of a Federal or State court of record or judge or clerk of probate court accepting applications, a designated municipal or county official, a designated postal employee at an authorized post office, an agent at a Passport Agency or a U.S. consular official at a U.S. Embassy or Consulate, if abroad.

If you are 16 years of age or older: The passport processing fee is $55.00 (US Currency). In addition, a fee of $30.00 (US Currency) is charged for the execution of the application. Your passport is valid for 10 years from the date of issue except where limited by the Secretary of State to a shorter period. Total fee is $85.00 US Currency If you are 15 years or younger: The passport fee is $40.00 (US Currency). In addition, a fee of $30.00 (US Currency) is charged for the execution of the application. Your passport is valid for 5 years from the date of issue except where limited by the Secretary of State to a shorter period. Total fee is $70.00 US Currency. Expedited Service: Add $60.00 (US Currency) per application & 2-Way Overnight Delivery. (Expedite fee does not include overnight delivery).

Mail In Application

You can do a Mail In Application if you can answer yes to next 5 statements.

  1. I can submit my most recent passport. Yes No
  2. I was at least 16 years old when my most recent passport was issued. Yes No
  3. I was issued my most recent passport less than 15 years ago. Yes No
  4. I have used the same name as on my most recent passport, OR I have had my name changed by marriage or court order & can submit proper documentation to reflect my name change. Yes No
  5. The old Passport is undamaged. Yes No

If you answered NO to any of the four statements above, STOP- You can not use this form. You must apply in person on application for DSP-11 by making a personal appearance before a passport agent, postal clerk or clerk of court authorized to accept passport applications.

If you are eligible for the Mail In Application you need to send the following with the application form:


Your most recent passport
Issued at age 16 or older in your current name (or see item Marriage Certificate below) & issued within the past 15 years. If your passport is mutilated or damaged, you must apply on the DSP-11.

A marriage certificate or court order if your name has changed
If the name you are currently using differs from the name on your most recent passport, you must submit a marriage certificate or court order showing all the change of name. The name change document must bear the official seal of the issuing authority. Uncertified copies or notarized documents cannot be accepted. All documents will be returned to you with your passport. If you are unable to document your name change in this manner, you must apply on the DSP-11 Application form by making a personal appearance at (1) A passport agency; (2) any Federal or State court of record or any probate court accepting passport applications; or (3) a Post Office which has been selected to accept passport applications

Passport fee of $55.00
Enclose the $55.00 (US Currency) passport fee in the form of a personal check or money order. DO NOT SEND CASH. Passport services cannot be responsible for case sent through the mail. If you desire special postage other than first class (registered, special delivery, etc) include the appropriate fee on the check or money order. The full name & date of birth of the applicant must be typed or printed on the front of the check. Make checks payable to Passport Services. Expedited service is available only in the United States. Expedited request will be processed in 3 workdays from receipt at a Passport Agency. This service is only for early departure, generally with proof of travel. The additional fee is $35.00 (US Currency)

Two recent (taken within the last 6 months) identical photographs with light, plain background

Issuance of a Passport to a Minor Under Age 14
